Low Income Subsidy (LIS) Plans
Im on a limited income. Is there any help for members with low incomes?
Yes, you need to complete an application to determine if you are eligible to receive this help. If you think that you may qualify for the Medicare Prescription Drug Coverage (Part D) low-income subsidy, complete the application. If you need a copy, or if you need help filling out the form, contact:

If you are eligible for low-income help from Medicare, Medicare will tell you what level of help you will receive.
Whether you are approved or denied the low-income subsidy, you still need to decide whether you want the Medicare prescription drug coverage (Part D). If you decide yes, you must let your health plan know that you want to enroll in its Medicare prescription drug coverage (Part D) effective January 1.
MVP follows Medicare's Best Available Evidence policy to determine your eligibility for extra help paying for your prescription drug costs. For more information on BAE visit Medicare's Centers for Medicare & Medicaid Services (CMS) website.
Where can I get more information on Medicare Part D prescription drug coverage?
There are many places for you to get more information:
- Visit Medicares website for beneficiaries: www.medicare.gov.
- Visit Social Security Administrations website entitled, Help with Medicare Prescription Drug Plan Costs at www.socialsecurity.gov.
- Call 1-800-MEDICARE (1-800-633-4227) to speak to a Medicare customer service representative. TTY users may call 1-877-486-2048.
- Call the Social Security Administration Office for low-income subsidy information at
